Cape Cod Healthcare Releases FY2019 Annual Strategic Grant Request for Proposals (RFP)
Cape Cod Healthcare is pleased to release its FY2019 Annual Strategic Grant Request for Proposals (RFP). This RFP is part of an annual competitive grant process focused on addressing the unmet health needs of the residents of Barnstable County. The FY2019 Annual Strategic Grant RFP focuses on two of the four health priorities identified in the 2017-2019 Cape Cod Hospital and Falmouth Hospital Community Health Needs Assessment (CHNA) Report and Implementation Plan:
• Chronic and infectious disease management
• Disease prevention and wellness
Grant proposals are required to address at least one of the following specific chronic or infectious diseases or disease prevention and wellness priorities, and meet funding guidelines related to vulnerable populations and measurable outcomes. The specific chronic and infectious diseases of significance and growing concern in Barnstable County based on disease incidence, prevalence and mortality include:
• Cancer
• Cardiovascular disease
• Alzheimer’s and Dementia
• Hepatitis C
• HIV/AIDS
• Tick-borne diseases
Disease prevention and wellness priorities identified through self-reported health risk factors, health indicator data and community input in Barnstable County include:
• Health screenings
• Physical activities and nutrition
• Elder health and wellness
• Caregiver support
In addition, CCHC encourages applicants to include program strategies that reduce social isolation and/or improve transportation for target populations impacted by the project. Social isolation and transportation are two key social determinants that impact the health and wellness of Barnstable County residents.
The FY2019 Annual Strategic Grants program will provide up to $250,000 total to support projects ranging from $15,000 to $30,000. The deadline for submission of strategic grant proposals is August 31, 2018. Grant selection will be finalized in November 2018 by the CCHC Community Health Committee. The funding period runs from January 1 – September 30, 2019. The Cape Cod Healthcare Community Benefits Annual Strategic Grants program is a part of a larger grants initiative that supports a broad range of local health-based projects including support for local community health centers. In addition to grant funding, CCHC’s Community Benefits efforts include hospital-based outreach, education and support programs, workforce development initiatives, charity care, regional health and human service partnerships, and employee volunteerism. In 2017, CCHC’s Community Benefits expenditures totaled more than $25 million in community health initiatives.
