Chronic Disease Self-Management Learning Opportunities

Chronic Disease Self-Management Program (CDSMP) helps adults with chronic diseases learn how to manage and improve their health. Through the Barnstable Prevention and Wellness Trust Fund, the Department is supporting two new educational opportunities on Cape Cod.

Workshop Series to become an “Active Self-Manager” with “My Life, My Health”:
A six-week workshop series for any adult who wants to “put life back in your life” begins on May 31, 2016.  Topics include Nutrition, Pain and Fatigue Management, Weight Management, and Appropriate Exercise. The workshops are hosted by the Mashpee Public Library and are sponsored by the Healthy Living Center of Excellence Cape and Islands, Healthy Living Cape Cod, the Cape Cod Foundation, and the Barnstable Prevention and Wellness Trust Fund. Training for “My Life, My Health” Program Leader:
Chronic Disease Self-Management Leader Training begins May 16, 2016.  The workshop series is for adults interested in learning how to facilitate Stanford University’s Chronic Disease Self-Management program known as “My Life, My Health”.  The four day training is hosted by Elder Services of Cape Cod and the Islands, and is sponsored by the Healthy Living Center of Excellence Cape and Islands, Healthy Living Cape Cod, the Cape Cod Foundation, and the Barnstable Prevention and Wellness Trust Fund.  Read more about the leader training…


With funding from the Commonwealth’s Prevention and Wellness Trust Fund (PWTF), the Barnstable County Department of Human Services formed the Barnstable Prevention Partnership which consists of three local community health centers and two local community organizations.  The Barnstable Prevention Partnership has created a referral and health education system that is based on the concept of self-management of health risks.  It is one of nine such partnerships funded by the PWTF that share over $40 million in grants to help fight chronic illness and improve health outcomes on the community level while reducing health care costs.
